FormWise

FormWise is a no-code platform that turns an expert's methodology into branded AI products — SmartForms, CoPilots, voice Experts and Toolsets — for agencies, coaches and consultants. Every output is a structured, editable document rather than a chat transcript.

Overview

What is FormWise?

FormWise is a no-code infrastructure layer for productising expertise: it takes what a specialist knows and packages it into branded AI products a client can use alone. The thesis is displayed plainly on the site — old bottleneck: code; new bottleneck: expertise.

Four product bricks sit on the platform. SmartForms are guided forms designed by the expert; answers travel through a chain of prompt nodes that reference the knowledge base, and the output is an Artifact rendered in a controlled template, editable and downloadable, which the user can then chat with to refine. CoPilots are goal-driven conversational agents: a main node carries identity, personality and objective, chained nodes handle research, coaching, classification and routing, parallel nodes run side by side, and webhook nodes push into a CRM; the agent produces a structured Artifact and can fire a webhook once the goal is reached. Experts are the newest brick — voice agents, text or two-way speech with a customer-supplied ElevenLabs key, a custom or catalogue voice, able to call the other tools. Toolsets are the branded portal that bundles the rest: the end user fills in personalisation fields once and every tool in the portal knows their context, while user-level Notebooks accumulate context from session to session.

Underneath sits the knowledge layer, called Notebooks on most pages and Business Brain on the platform page, ingesting PDF, CSV, TXT, transcripts, Notion pages, website URLs and YouTube transcripts. The publisher describes four context layers: prompt and workflow logic, organisation Notebooks, end-user personalisation, and live data integrations.

That combination is what separates it from a thin model wrapper. Claude, OpenAI (GPT-5 access on Agency), Gemini and Mistral come with credits included, so no API key is needed, though BYOK is optional. Deployment runs through a GoHighLevel sub-account iframe, a Kajabi lesson, WordPress, any website, or a standalone Login Mode portal, and Stripe bills the operator's own clients. The publisher is Formwise LLC, whose leadership is presented by first names only; V2 is sold as a full rebuild after the reliability problems the team publicly acknowledges in V1.

What it does

  • Turn a methodology into a branded SmartForm that returns an editable, downloadable Artifact
  • Run goal-driven CoPilots that research, coach, classify and route, then fire a webhook into the CRM
  • Load PDFs, transcripts, Notion pages, site URLs and YouTube transcripts into Notebooks as the knowledge layer
  • Describe the business to the Agent Builder Wizard and get a working draft without writing a prompt
  • Bundle tools into a white-labelled Toolset portal with end-user personalisation fields
  • Embed anywhere: HighLevel sub-account iframe, Kajabi lesson, WordPress, standalone link or Login Mode portal
  • Charge end clients directly through Login Mode and Stripe, as a one-off, a subscription or email access

When to use FormWise / When not to

A quick filter to help you decide if FormWise is the right fit.

When to use FormWise

  • GoHighLevel agencies looking to differentiate beyond the snapshot everyone else resells
  • Coaches and consultants whose methodology is trapped in PDFs, slide decks and one-to-one calls
  • Course creators embedding guided AI tools inside a Kajabi lesson or a HighLevel membership
  • Subject-matter experts who would rather sell access to a branded portal than sell their time
  • Non-technical operators: the Agent Builder Wizard drafts a working tool from a plain conversation

When not to use FormWise

  • Teams wanting a permanently free tool: there is none, and the entry point is a 129 USD/month subscription after a 7-day trial
  • Mobile-first users: there is no iOS or Android application, the product is web only
  • Developers expecting published API documentation: API and webhooks are sold on the Pro plan but no docs page exists
  • Organisations bound by EU data residency: hosting is exclusively in the United States, with no GDPR mention, no DPA and no subprocessor list
  • Non-English-speaking teams: the interface and the whole site are in English, and Toolset translation into 70+ languages is Enterprise-only
Get started

How to use FormWise

A typical end-to-end flow, from setup to results.

  1. Start the 7-day free trial at the sign-up page, with no sales call required first
  2. Load the methodology into Notebooks: PDFs, call recordings, transcripts, Notion pages and website URLs
  3. Describe the business to the Agent Builder Wizard, which asks a few clarifying questions then drafts a first SmartForm or CoPilot
  4. Refine the draft in the manual editor: prompt nodes, classification, extraction, conditional branches, parallel nodes and webhooks
  5. Set the personality, the questions and the output format, then test in the preview pane
  6. Build a voice Expert in five steps: name it and set guardrails, give it a goal, pick its Notebooks, tune voice and tone, publish it
  7. Bundle the tools into a branded Toolset and define the end-user personalisation fields
  8. Connect live client data through the integrations connector, for reading and writing
  9. Deploy: HighLevel sub-account iframe, Kajabi lesson, WordPress, standalone link, or your own domain in Login Mode
  10. Use the onboarding call included with every plan and the weekly office hours to unblock a build in progress

Pros & Cons

Pros

  • No API key to obtain: model credits are included in every plan
  • Full white labelling from the entry plan, so the publisher stays invisible to the end client
  • A human onboarding call is included on every plan, during the trial as well
  • Four complementary bricks, including a voice agent, which is uncommon among no-code competitors
  • The output is a structured, downloadable document rather than a chat thread
  • Built-in monetisation through Login Mode and Stripe, with native GoHighLevel integration from the Agency plan
  • Multi-model access (Claude, OpenAI, Gemini, Mistral), a written 7-day refund policy and a 99.9% uptime target

Cons

  • No permanently free plan; the monthly entry point is 129 USD per month
  • Misleading price display: the pricing page opens on annual billing and shows 107 USD before the real monthly 129 USD
  • Three different Pro prices coexist on the site on the same day: 99, 107 and 129 USD
  • Legal documents lag the product: the main text is dated 6 October 2024, still describes a form builder and mentions only OpenAI, and the User Access Policy links twice to a dead privacy policy URL
  • No ownership clause for customer-created content, no GDPR mention, no DPA, no subprocessor list and no EU representative
  • United States hosting only, with no European option, and no published API documentation although the API is sold as a Pro feature
  • Thin corporate transparency (no postal address, no full leadership names, no identifiable beneficial owner), no mobile app, real-time voice requiring the customer's own ElevenLabs key, and a publicly acknowledged reliability record in V1
Pricing

Pricing & Plans

There is no permanently free plan. Access starts with a 7-day free trial on the Pro and Agency tiers, onboarding call included. The lowest paid entry point is 129.00 USD per month for Pro on monthly billing, with Agency at 229.00 USD per month. Under an annual commitment the same plans come down to the equivalent of 107.00 USD and 190.00 USD per month, billed 1,284.00 USD and 2,280.00 USD per year, with advertised savings of 264.00 USD and 468.00 USD per year. Those annual equivalents are what the pricing page displays by default, so the first amount a visitor sees is not the monthly price. The commitment discount is permanent and carries no stated end date. Enterprise is quoted on request through hello@formwise.ai. Credits are included (5,000 per month on Pro, 10,000 on Agency, granted a year in advance on annual plans), overage runs through an On-Demand Usage meter with a customer-set spending cap, there are no per-user fees, and refunds are available within 7 days of the last billing date. Note that the meta description and the pre-rendered version of the site still advertise Pro from 99 USD per month.

Trust & Privacy

Data, GDPR & hosting

A consolidated view of how FormWise handles your data.

GDPR overview

The word GDPR appears nowhere on the site, on none of the fourteen pages reviewed. There is no EU representative under Article 27, no DPO and no dedicated data protection address; the single contact for any privacy question is hello@formwise.ai. No DPA is published or offered, no subprocessor list exists, and no standard contractual clauses or international transfer mechanism are mentioned. The only compliance wording is generic: We adhere to the relevant data protection regulations, and customers may request information about our compliance standards (User Access & Fulfillment Policy, 19 June 2026). The 6 October 2024 document lists none of the data subject rights — access, rectification, erasure, portability, objection — and mentions only account deletion. Hosting is declared exclusively in the United States. The minimum age of 13 is the US COPPA threshold, not a GDPR one.

Who owns the data?

The combined Privacy Policy / Terms Of Service dated 6 October 2024 contains no clause at all on ownership of the content customers create: SmartForms, CoPilots, Toolsets and Artifacts are simply never addressed. What the document does say is that Formwise LLC stores user session data, tool data and prompts on its own servers, and that it may screen prompts and inputs for usage-policy compliance. It collects names, emails, billing details, generated content, the forms created and the data they capture, plus IP, browser, device and OS data. Customers may not use the platform to build a competing product or reverse-engineer it. The marketing line Your methodology stays yours is a promise on a product page, not a contractual term.

Reuse rights

The contract lists broad purposes: delivering and improving the service, personalisation, troubleshooting, communication, answering requests, producing usage analytics and statistics, and meeting legal obligations. Disclosure is permitted with consent, to third-party service providers, on legal request and to protect the publisher's rights — and no subprocessor list is published. On the homepage FAQ the publisher states that Notebook knowledge stays inside FormWise, that it does not share customer data with model providers for training, and that end-user context in Toolset Notebooks is scoped per user and per Toolset. That commitment is narrow: it covers sharing with model providers only and says nothing about internal use by FormWise itself, and no opt-out mechanism is documented anywhere. As for the customer reusing what the platform produces, the terms are silent: beyond the ban on building a competing product or reverse-engineering the platform, nothing grants or restricts reuse of the Artifacts generated. Note also that the 2024 text still refers exclusively to OpenAI while the 2026 product advertises Claude, Gemini and Mistral.

Data retention & training

Retention summary
The publisher keeps information only for as long as necessary to fulfil the purposes for which it was collected, or as required by applicable law. Deleting an account removes the information from its servers and its backups, except where legal obligations or the protection of the publisher's rights require otherwise. When a trial is not converted, the account stays accessible for thirty days, then the data is removed. Beyond that thirty-day window no retention period is quantified: nothing is stated for usage data, logs or generated Artifacts. Payment methods are not stored, being handled by third-party processors, and customer-supplied OpenAI API keys are encrypted and, the publisher says, inaccessible to it. No zero-retention policy, no on-demand purge option and no anonymisation practice are documented.

Hosting summary

Formwise LLC states in its combined Privacy Policy / Terms Of Service that our servers, where all data collected and processed through our Service is stored, are located in North America, specifically in the United States, adding that no data is hosted abroad. There is no European data residency option and no choice of region. The public site sits behind Cloudflare (IP 104.21.66.73, anycast), but that is a CDN edge, not the storage location: the applicable jurisdiction is the United States. No hosting certification is cited, with neither SOC 2 nor ISO 27001 nor HIPAA appearing anywhere on the site. No subprocessor list is published; the terms refer to unnamed third-party service providers. The only technical measure described is the encryption of customer-supplied OpenAI API keys, which the publisher says it cannot access. For a European buyer, this means US jurisdiction with no disclosed transfer mechanism, no DPA and no Article 27 representative.

Things to keep in mind

Risks and trade-offs to weigh before adopting FormWise.

  • The pricing page opens on annual billing: the first amount you see, 107 USD, is not the monthly price of 129 USD
  • Three Pro prices coexist on the same day — 99 USD in the meta description, the JSON-LD FAQ and the pre-rendered HTML, 107 USD annual, 129 USD monthly — and the saving badge reads 708 USD per year on the homepage against 468 USD per year on the pricing page
  • The terms say nothing about who owns the Toolsets, SmartForms and Artifacts a customer builds, so the ownership of the asset you are packaging rests on a marketing promise rather than a clause
  • The User Access Policy links twice to a privacy policy address that does not exist, and the main legal text, dated 6 October 2024, still describes a form builder running on OpenAI
  • An unusual clause has the user agree not to dispute any charges
  • The no-training commitment covers sharing with model providers only and says nothing about internal use; no opt-out is documented
  • Claims shift between pages: integrations counted as 300+, 1,000+ and 10,000+; the free HighLevel sub-account valued at 97 USD per month; native GoHighLevel integration, central to the agency pitch, absent from the Pro plan; and the Wizard capped monthly at 20 tools on Pro and 125 on Agency
Setup

Setup & Integrations

Technical difficulty

Low on the guided path. No coding is required, no API key has to be obtained since credits are included, and the Agent Builder Wizard drafts a SmartForm or CoPilot from a conversation, without writing prompts or picking a model. The site claims five minutes to a first product, and every plan includes an onboarding call plus weekly office hours. Higher for advanced work: the manual node editor covers classification, extraction, conditional branches, parallelism and webhooks; embedding in a GoHighLevel sub-account means configuring an iframe menu link; real-time voice requires an ElevenLabs key; a custom domain is Enterprise-only.

FAQ

Frequently asked questions

How is FormWise different from using ChatGPT?
FormWise builds a branded product on your own methodology, deployed inside your client's workflow and returning a structured deliverable. The client logs into your portal, not into OpenAI's.
What is the real monthly price?
On monthly billing, Pro costs 129 USD per month and Agency 229 USD per month. The 107 USD and 190 USD figures shown first on the pricing page are the monthly equivalents of an annual commitment.
Is there a free plan?
No. There is no permanently free tier, only a 7-day free trial on Pro and Agency, which includes a human onboarding call and is not charged.
What happens when the trial ends?
You choose a plan. If you decide not to continue, the account stays accessible for thirty days in case you change your mind, then the data is removed.
Do I need to know how to code?
No. The Agent Builder Wizard generates a working draft from a conversation, and the manual node editor stays available when you want more control.
Do I need my own API key?
No. Credits are included on every plan and cover Claude, GPT-5, Gemini and Mistral. Bringing your own key remains optional, except for the real-time voice Experts, which require your own ElevenLabs key.
Where does my data live, and is it used to train models?
The servers are in the United States. The publisher states that it does not share customer data with model providers for training. That statement covers model providers only, and no opt-out mechanism is documented.
Where can I deploy a Toolset?
In a GoHighLevel sub-account or membership, a Kajabi lesson, a WordPress site, any other website, or as a standalone portal in Login Mode.
Can I charge my own clients through the platform?
Yes. Login Mode combined with Stripe supports one-off purchases, subscriptions or email-based access, per tool or per Toolset.
Is FormWise GDPR compliant?
The site never claims it. The word GDPR appears nowhere, there is no DPA, no subprocessor list and no EU representative, and hosting is in the United States only.
Conclusion

Should you pick FormWise?

FormWise makes a clear, differentiated promise to a precise segment: GoHighLevel agencies, coaches and consultants who want to sell a product rather than hours. The product depth is real — four tool types, a document-grounded knowledge layer, live data connectors, built-in monetisation and white labelling from the entry plan — and with model credits included and a human onboarding call on every plan, the technical barrier is genuinely low. For an operator whose expertise currently lives in PDFs and one-to-one calls, that combination is the argument.

Two reservations weigh heavily. The legal apparatus lags the product: the main document dates from 6 October 2024, still describes a form builder running on OpenAI, contains no ownership clause for the SmartForms, Toolsets and Artifacts a customer creates, never mentions the GDPR, and the User Access Policy points twice at a privacy policy URL that does not exist. And the pricing presentation leads with the annual equivalent, so three different Pro prices — 99, 107 and 129 USD — can be read on the same site on the same day.

Hosting is exclusively American with no European residency option, a blocker for any organisation under an EU data residency constraint. The publisher is also thin on identity: no postal address, no full leadership names, no identifiable beneficial owner behind Formwise LLC.

To its credit, the team publicly owns the failure of V1 and sells V2 as a rebuild, an honesty signal but also a reliability history worth watching. The verdict: a strong fit for an agency or expert who wants to package and resell their method quickly and is indifferent to data residency, and a poor fit for any buyer who needs contractual clarity on data and content ownership before signing.
