Align X-ray Insights is a software-only computer-assisted detection device (CADe) for dental radiology, published by Align Technology, the company behind Invisalign and the iTero scanner range. It reads panoramic, periapical and bitewing radiographs and uses deep learning networks trained for classification, detection and segmentation to locate and label teeth, then to detect and document both pathologies and non-pathological structures.
Eleven findings are covered: caries, periapical radiolucencies, periodontal bone loss, calculus, fillings, crowns, bridges, implants, root-canal fillings, impacted teeth, and the proximity of lower molars to the mandibular canal. Which ones are active depends on the image type, and the vendor warns that available detections vary from market to market. Results appear as colour-coded annotations laid directly over the radiograph and mirrored on an interactive tooth chart, so the same screen serves both the clinical reading and the conversation with the patient. Two focused modes extend the default view: Caries Pro pinpoints the location and depth of carious lesions on bitewings, and Perio Pro highlights periodontal bone loss on panoramics. A High Sensitivity tool lets the clinician re-read caries and periapical radiolucencies with the threshold raised.
The workflow has four steps: automatic retrieval of radiographs and patient metadata from the practice server on the Pro plans, standardised analysis of the image, review of the detections with the drawing and tooth-operation tools, then confirmation of the findings and generation of a detailed report. Everything runs in a browser through a portal; there is no mobile application and no public API.
The technology did not start at Align. It grew out of a 2017 idea from Professor Falk Schwendicke and Dr Joachim Krois of Charité – Universitätsmedizin Berlin, was built from 2018, incorporated as dentalXrai GmbH in 2020, and was acquired by Align Technology in 2022 before launching in Europe in 2023 as a limited market release. The device falls under the Medical Device Directive in the European Union, the United Kingdom and Switzerland, and is cleared in Canada. It is intended for dentists working on permanent dentition and must never be used on its own to make or confirm a diagnosis.

GDPR overview
Align never claims GDPR compliance in so many words on any page or document linked from the site; the only occurrence of the acronym is in a Costa Rica country annex that is unrelated. What is documented is concrete nonetheless: a Data Protection Officer is appointed where required and reachable at privacy@aligntech.com, international transfers rely on adequacy decisions, standard contractual clauses or Align's Binding Corporate Rules, and both the controller and processor Binding Corporate Rules documents are published. Who owns the data?
The governing documents are Align Technology's general website terms and its global Websites and Apps Privacy Notice, last updated on 25 June 2025; neither is specific to this software. For patient data, Align states that it normally acts on the instructions of the treating clinician, who decides what is collected and how it is processed and remains the patient's first point of contact.
