What is GestAI?

GestAI is an Italian practice-management platform for mental health professionals, published by 4Mee S.r.l., a company registered in Bomporto near Modena and described on the site as an innovative startup. It positions itself as the first Italian practice-management system built around an AI agent for psychotherapy, and it splits into two halves meant to work as one.

The first half is ordinary practice administration, done for the Italian market: an appointment book with single and recurring slots, online booking links, availability and automatic reminders; a digital clinical record holding case history, session diary, tests, consents and attachments; digital signature for consent forms; payment links; private invoicing; electronic invoicing through the SDI system; credit notes; tax settings; an accountant portal; and a statistics dashboard with filterable charts and data export.

The second half is the AI assistant, deliberately vertical rather than a general chatbot. Sessions can be recorded or uploaded; the platform transcribes them speaker by speaker, applies diarisation, and returns a clinical summary highlighting key themes and emotional markers. From there it builds a Quadro Clinico, an automatic snapshot of the patient, and offers two conversational surfaces: one scoped to a single session, one covering the whole treatment path. The assistant follows the context of the page in view — a session, a patient file, a document, or a general chat on theory and practice organisation — and retrieves from transcripts, notes and documents as needed.

Usage is metered by tier: 10, 50 or 100 analysable sessions a month, 600K, 1,500K or 3,000K AI chat credits, and 10, 50 or 100 GB of storage, with an advanced AI engine reserved for the top tier. Unlimited patients, unlimited invoices, European hosting, encryption and backups are included at every level.

The vendor is explicit about limits: AI output is organisational and documentary support, never diagnostic or autonomously decisional, and the therapist remains solely responsible. The homepage claims 85% less time on paperwork and three extra hours a week for patients — vendor figures, published without a supporting source.

GDPR overview

Implementation is detailed rather than declarative. The privacy notice (version 2.3, updated 1 July 2026) is drafted under Articles 13-14 GDPR and names 4Mee S.r.l., Via Carrate 42, Solara di Bomporto (MO), VAT 04262930367, Modena Companies Register, REA MO-456230. No DPO is appointed: the company documented an Article 37 assessment and designates an internal privacy officer reachable at privacy@gestai.it. A DPIA under Article 35 covers special-category health data and the use of AI, with an executive summary available to professional users on written request. An Article 28 DPA applies automatically on acceptance of the terms, and a named subprocessor list is published with 30 days' notice before any change. Rights under Articles 15-22 are answered within 30 days, complaints going to the Garante. Article 50 of the EU AI Act is addressed separately.

Who owns the data?

Two roles run in parallel. For the practitioner's own data — identity, billing, technical logs — 4Mee S.r.l. acts as data controller. For patient data the practitioner is the sole controller and 4Mee is a processor under Article 28 GDPR, acting only on instruction and never deciding purposes or means. The terms state that uploaded data remain the property of the user or of the rightful holders, the vendor receiving only a limited licence to run the service. Intellectual property in the platform itself stays with 4Mee. One clause deserves attention: feedback, suggestions and ideas submitted by users are assigned to the vendor free of charge and without limitation.

Reuse rights

Patient data are processed only to deliver the functions the practitioner asks for. Four separate documents — the privacy notice, the terms, the DPA and the subprocessor list — state that these data are never used to train, fine-tune or improve any AI model, whether the vendor's or a third party's, and are never sold, profiled or used for marketing. AI processing runs on Amazon Bedrock and Bedrock Data Automation for transcription, diarisation, summarisation and contextual retrieval. Under Article 22 GDPR no fully automated decision producing legal effects is taken: every AI output is support material that the clinician must review before use. Service improvement relies on aggregated or anonymous data only. Analytics and advertising tools run on the public marketing site alone and never inside the application, and Google Calendar data are excluded from all AI features. The applicable privacy notice is version 2.3, in force since 1 July 2026.

Data retention & training

Retention summary
Account data are kept for the life of the contract and deleted within 30 days of definitive closure, subject to legal obligations. Billing and transaction records are kept for ten years under Article 2220 of the Italian civil code. Application logs run six to twelve months, security logs 24 months, and support correspondence up to 24 months. Patient data are deleted within 30 days when the account is closed on request. An account left in the Free Limitato state is not purged automatically: data stay for the duration of the contractual relationship, and the vendor sends an annual email reminder offering export or definitive closure. On termination, 30 days are allowed for recovery before deletion or anonymisation. Practitioners must export their own copies beforehand. If GestAI ever shuts the service down, it commits to 60 days' notice.

Hosting summary

Clinical components are hosted inside the EU/EEA. Supabase provides the database, authentication, storage and edge functions from the West Europe (Frankfurt) region, with its legal seat in Singapore and extra-EU transfers covered by the EU Standard Contractual Clauses 2021/914. AWS handles infrastructure and AI processing — Bedrock and Bedrock Data Automation — in configured EU regions such as eu-central-1 Frankfurt, contracted through Amazon Web Services EMEA SARL in Luxembourg. Stripe keeps the EU as primary with a United States operational backup under the EU-US Data Privacy Framework and SCCs. Resend, used for transactional email, hosts in the United States with an EU region described as under evaluation, and handles no health data. Hotjar stays entirely within the EU. Google, Meta and LinkedIn operate on the public marketing site only. The vendor states that no patient health data is transferred outside the EU. Cloud servers carry ISO 27001, 27017 and 27018 certification — the providers' certifications, not 4Mee's.   • Leaning on generated summaries can erode the clinician's own habit of listening and note-taking; the vendor itself insists every output must be reviewed
  • Recording a therapy session changes the therapeutic frame: consent must be obtained patient by patient, and that responsibility sits with the practitioner
  • Transcription and diarisation errors carried into a clinical record are hard to spot once the note has been filed
  • AI readings of emotional trends, stalls and regressions can be taken for objective data when they are statistical suggestions

Should you pick GestAI?

GestAI makes a narrow bet and documents it unusually well. Rather than bolting a chatbot onto a generic scheduling tool, it builds the AI into the clinical record itself: sessions are transcribed speaker by speaker, summarised, and made searchable through an assistant that knows whether you are looking at one session, one patient or the whole practice. Around that sits a full Italian administrative stack — SDI electronic invoicing, Sistema Tessera Sanitaria, an accountant portal, digital consent signatures — that no international competitor is likely to replicate.

The legal apparatus is the surprise. A young company with 10,000 EUR of share capital publishes a versioned privacy notice, an Article 28 DPA, a named subprocessor list with country-by-country server locations, and a documented DPIA.
